Authors report a case of Klippel-Feil syndrome (fusion of C2-3, C5-6) associated with bilateral Sprengel deformity of scapula. This 38 years old male case presented with short neck, limitation of neck motion, low back hair line and winged scapula. MDCT and 3D reconstruction showed cervical neural foraminal stenosis, scoliosis of cervical spine, spina bifida, upward rotatory displacement of hypoplastic left scapula, skull base anomaly, omovertebral bone, and open occipital fontanelle. Secondary degenerative changes were noticed on cervical spine adjacent to fused vertebrae.
